Deliver skilled nursing care and comprehensive assessments to PACE participants within their homes to support independence and well-being. Collaborate with an interdisciplinary team to implement individualized care plans and monitor health status changes.
Requires a nursing degree (Associate or Bachelor's) and a current Registered Nurse license in Louisiana. A minimum of one year of nursing experience and a valid driver's license with reliable transportation are required.
