Fee-for-Service Physical Therapists are required to: Work a minimum of 2 shifts per pay period (every 2 weeks) Attend the full first week of orientation, Monday through Friday from 8:30 AM – 4:30 PM If no prior CHHA experience, commit to training 2 days per week until fully oriented and independently managing
Review patient charts and medical history Develop a personalized treatment plan Teach therapeutic exercises and stretches to improve patient’s conditions Use hands-on therapy to promote healing Maintain patient records by keeping track of goals and progress while adapting treatment plans accordingly Work closely with a supportive patient care team including physicians, nursing, social services, pastoral care, home health aides and others for your assigned caseload. New York State PT license Minimum of one year of recent hands on experience as a Physical Therapist. Valid New York State Driver’s License and reliable transportation
