Position Summary

The Occupational Therapist provides evaluation and treatment services for patients referred by physicians across outpatient, home health, and hospice settings. This role develops individualized plans of care, collaborates with interdisciplinary teams, and delivers compassionate, evidence-based therapy that supports patients in achieving their highest level of independence and function. What You'll Do

Responsibilities

include: Provide comprehensive evaluation and treatment services for patients referred to for Occupational Therapy care. Develop individualized care plans based on patient evaluations and clinical findings. Deliver compassionate, evidence-based therapy focused on improving patient function and independence. Document all patient care accurately and complete charge entry in a timely manner according to department policies. Collaborate closely with physicians and interdisciplinary rehabilitation team members to support coordinated patient care. Report directly to the Director of Rehabilitation. Participate in departmental planning initiatives, including budget and resource planning activities. Contribute as an active member of the Occupational Therapy and Rehabilitation teams. Support department and organizational quality improvement initiatives and performance projects. Participate in equipment training, organizational safety programs, and fire safety education activities.

Qualifications
