The Housing Specialist links residents to community programs and leads multi-disciplinary assessment teams to ensure service coordination. They are also responsible for facilitating care conferences and coordinating social, educational, and recreational programs for residents.
Candidates should have at least two years of experience in human services, preferably working with the elderly. A bachelor's degree in social work or a related field is preferred, along with proficiency in Microsoft Office.
