Responsibilities

• Discharge Planner We are seeking like-minded Discharge Planner to join our growing team!
• Quail Run Hospital is a 102-bed, acute care psychiatric hospital located in Phoenix, Arizona.
• Quail Run features individual units for adolescents, adults, and seniors, and offers inpatient acute care, partial hospitalization, and intensive outpatient programs.

• This includes discharge planning, placement of the patient, aftercare needs, advocacy needs, housing referrals, and entitlement needs.

Qualifications

• Job Requirements Bachelor’s degree from an accredited college or university in Social Work or related mental health field required.
• A minimum of two (2) years direct clinical experience in a psychiatric or mental health setting and direct experience in patient/family assessment required.
• Experience in working with programs and outside referrals preferred Level 1 Arizona Fingerprint Clearance card or be able to obtain within 30 days of hire.
