An Associate's or Bachelor's degree in Social Work, Human Services, or a related field is required. Candidates must be able to perform home visits in specific Pennsylvania counties and pass multiple background clearances.
The Nursing Home Transition (NHT) Coordinator plays a vital role in helping individuals safely and successfully transition from nursing facilities back to community-based living. This position works closely with participants, families, service coordinators, healthcare providers, and external agencies to ensure services and support are in place before, during, and after discharge. The NHT Coordinator is responsible for adhering to all program requirements, maintaining timely and accurate documentation, and providing consistent participant support throughout the transition process. We provide comprehensive on-the-job training and mentorship to support employees as they develop the skills needed to succeed in this role. Education & Experience Associate's or Bachelor's degree in Social Work, Human Services, Nursing, Psychology, Sociology, or a related field required (Bachelor's degree strongly preferred). Recent college graduates are encouraged to apply. Previous experience in care coordination, case management, healthcare, or human services is preferred but not required for entry-level candidates. Knowledge of Medicaid Waiver programs, Long-Term Services and Supports (LTSS), and Home and Community-Based Services (HCBS) is a plus. What AUCP Offers YOU Starting
