Position Summary

The Nursing Home Transition (NHT) Coordinator plays a vital role in helping individuals safely and successfully transition from nursing facilities back to community-based living. This position works closely with participants, families, service coordinators, healthcare providers, and external agencies to ensure services and support are in place before, during, and after discharge. The NHT Coordinator is responsible for adhering to all program requirements, maintaining timely and accurate documentation, and providing consistent participant support throughout the transition process. We provide comprehensive on-the-job training and mentorship to support employees as they develop the skills needed to succeed in this role. Education & Experience Associate's or Bachelor's degree in Social Work, Human Services, Nursing, Psychology, Sociology, or a related field required (Bachelor's degree strongly preferred). Recent college graduates are encouraged to apply. Previous experience in care coordination, case management, healthcare, or human services is preferred but not required for entry-level candidates. Knowledge of Medicaid Waiver programs, Long-Term Services and Supports (LTSS), and Home and Community-Based Services (HCBS) is a plus. What AUCP Offers YOU Starting

Hourly Rate

$21/hour, with opportunities for performance-based increases Flexible Schedule No On-Call Requirements 15 Agency Paid Days Off (including 11 holidays) 18 PTO Days Paid Training Period Mileage Reimbursement Comprehensive On-the-Job Training and Mentorship Tuition Reimbursement Opportunities Employees become eligible for tuition reimbursement after one year of employment to support continued education and professional growth Medical Insurance Company Paid Dental Insurance Company Paid Vision Insurance Company-paid life insurance with the option to add additional coverage for you and your family Up to 5% Match for 401(k) Key Responsibilities Serve as the primary contact for nursing home residents transitioning into community-based living through the NHT program Coordinate transition plans with participants, families, healthcare teams, housing providers, and community agencies Arrange housing and essential support services to ensure successful discharge and community reintegration Monitor participant progress and maintain accurate documentation in compliance with all regulatory and organizational standards Support outreach, quality assurance initiatives, mentorship activities, and regional travel as needed Skills & Knowledge Experience working with individuals with disabilities or special needs preferred Strong skills in assessment, planning, and participant-centered service delivery Excellent organizational, communication, and documentation skills Ability to work effectively as part of an interdisciplinary team Familiarity with community support systems, housing resources, healthcare, and human services Willingness to learn and grow within the organization Work Environment Frequent exposure to outdoor elements and varying weather conditions Occasional driving in poor weather conditions may be required Noise levels are typically minimal to moderate Employment is contingent on the successful completion of the following clearances
