Provide compassionate, patient-centered nursing care and comprehensive assessments for hospice patients in their homes. Develop individualized care plans and collaborate with an interdisciplinary team to ensure quality end-of-life care.
Requires a current Texas RN license and a minimum of two years of hospice nursing experience. Proficiency in English and Spanish is preferred, along with a valid driver's license and reliable transportation.
Provide compassionate, patient-centered nursing care to hospice patients in their homes Perform comprehensive assessments, including pain and symptom management, psychosocial support, and ongoing evaluation of patient needs Develop and implement individualized care plans in collaboration with the interdisciplinary team, patients, and families Administer medications and treatments according to physician orders and hospice protocols Educate patients and caregivers regarding disease progression, symptom management, medications, and end-of-life care Provide emotional support and guidance to patients and families throughout the hospice journey Collaborate closely with physicians, social workers, chaplains, aides, and other team members to ensure continuity of care Maintain accurate and timely clinical documentation in compliance with hospice regulations and agency standards Participate in bereavement support and family follow-up as appropriate Stay current with hospice best practices through ongoing education and professional development
Current and unrestricted Texas RN license Minimum of 2 years of hospice nursing experience required Strong clinical assessment and case management skills Experience with pain management, symptom control, and end-of-life care Bilingual English/Spanish preferred Proficient with electronic documentation systems, including Axxess or other hospice EMR platforms Comfortable using email, Slack, and other communication platforms Excellent verbal and written communication skills Compassionate, dependable, and team-oriented professional Knowledge of Medicare Conditions of Participation and Joint Commission standards preferred Reliable transportation and valid driver’s license required Why Join Pacific Hospice?
