The Weekend RN performs initial and ongoing physical assessments to determine hospice care needs and manage symptoms. They are responsible for triaging calls, modifying treatment plans, and coordinating with the interdisciplinary group to provide end-of-life care.
Candidates must hold a current Georgia registered professional nurse license and CPR certification. A minimum of 2 years of recent hospice experience and a valid driver's license with a reliable vehicle are required.
Description Shift: 10-hour shift, Saturdays and Sundays

Complete an initial assessment of patient and family to determine hospice care needs; provide a complete physical assessment and history of current and previous illness Use health assessment data to determine nursing diagnosis Triage calls and assist patient/primary caregiver with problem resolution; counsel the patient and family in meeting nursing and related needs Make visits for symptom management and support as needed Modify patient treatment plans as indicated by patients' responses and conditions Maintain current, accurate, detailed reports in accordance with policy Monitor, record and report symptoms and changes in patients' condition to the IDG Provide health care instructions to the patient as appropriate per assessment and plan Obtain and receive physician orders as required Pronouncement of patients Other duties as assigned
Current license to practice as a registered professional nurse in Georgia Current and continued CPR certification 2 years of recent hospice experience Good verbal and written communication and problem solving skills Responsible valid driver’s license Reliable, insured, automobile for making home visits required
