Case Associate - LPN/RN

Job Description

Summary: The Hospice Case Associate (RN or LPN) is responsible for providing direct patient care. Responsibilities include following the plan of care, providing treatments, working collaboratively with the team to meet positive patient care outcomes, and performing case management duties.

Essential Job Functions/Responsibilities

• 1.
• Provides direct patient care as defined by the State of Washington.
• 2.
• Implements current nursing practice following a comprehensive assessment and the plan of care.
• 3.
• Provides accurate and timely documentation of patient services to reflect the plan of care.
• 4.
• Assesses and provides patient and family/caregiver education and information pertinent to diagnosis and self-care.
• 5.
• Participates in the coordination of hospice services, appropriately reporting identified needs to the interdisciplinary group.
• Examples include hospice aide, OT, PT, MSW, ST, Dietitian, or Clinical Manager.
• 6.
• Uses equipment and supplies effectively and efficiently.
• 7.
• Provides appropriate pain/symptom management.
• Evaluate the patient’s response to treatments/medications.
• 8.
• Participates in personal and professional growth and development.
• Also participates in quality assessment performance improvement teams and activities.
• 9.
• Performs case management duties, including coordinating care plans, facilitating communication among the care team, and ensuring continuity of care.
• 10.
• Assists with discharge planning and follow-up care.
• 11.
• Performs other duties as assigned by the registered nurse.

Position Qualifications

• 1.
• Registered nurse or licensed practical nurse with current licensure to practice nursing in the state of Washington.

• Community health/hospice or medical/surgical experience is preferred.
