Occupational Therapist/ Home Care Case Manager Greater Cincinnati, Ohio Area Salaried Position General Summary/Responsibilities: The Occupational Therapist evaluates and then directly provides or supervises the provision of occupational therapy services to patients in their homes or residences.
When functioning in the capacity of the Home Care Case Manager (HCCM), the Occupational Therapist is responsible for the provision and coordination of all components of a patient’s home care services.
The Occupational Therapist provides care as ordered by the physician, follow policies, procedures, and conditions of participation, and regularly reports to their supervisor.
The incumbent facilitates the development and implementation of the plan of care as outlined by the physician.
In addition, the incumbent is responsible for all required documentation related to the care they provide and communicates pertinent information regarding the patient to all appropriate health care team members.
