Conducts psycho social assessment in order to evaluate patients current and future concerns.
Evaluates patient's previous living circumstances in context of current medical status to develop an appropriate discharge plan.
Minimizes patient anxiety concerning illness and facilitates post- hospital referrals.
Interacts with and supplements the health care team in order to enhance the social, emotional and psychological care of the patient/family/significant others.
Willing and able to travel to multiple worksites.
MSW is required from an accredited school of Social Work.
Minimum of 1 year experience working in a hospital environment is preferred. Excellent customer service skills required.
BLS required.
