The Advance Practice Provider delivers comprehensive primary medical care to home-bound older adults and engages in interdisciplinary care planning. They are responsible for evaluating patient care assessments and providing education and support to patients and their families.
Candidates must be a Nurse Practitioner or Physician Assistant with relevant experience and a current license to practice in New York State. Preferred candidates will have at least two years of experience in geriatrics, primary care, or related fields.
Salary Range: $115,000-$122,000 plus potential for quarterly productivity incentive bonuses
(Pay information will vary depending upon relevant experience for the position)
Full Time 40 hours Monday-Friday
The Physician House Calls (PHC) Physician Assistant / Advanced Practice Provider (APP; Nurse Practitioner (NP) or Physician Assistant (PA)) provides comprehensive primary medical care for the Physician House Calls (PHC) program to home bound older adults in their own residence under the guidance of the Vice President of Medical Services and Chief Medical Officer in collaboration with other members of the medical and interdisciplinary team. This position requires a high degree of independence, personal responsibility, and collaboration with other members of the interdisciplinary team, patients, their caregivers/family, and other service providers. This program aims to bring person-centered care to frail older adults in their place of residence and minimize the risk of requiring emergency services and hospitalization.
Nurse Practitioner (Master or Doctorate degree or equivalent) in Geriatrics, Family or Adult primary care. Physician Assistant (Master degree or equivalent) with relevant experience and current, unrestricted license to practice in New York State. Current, unrestricted New York State Driver License, automobile and automobile insurance required as well as current and appropriate required certification.
Preferred candidates will have a minimum of two years of experience as an APP, ideally with a background in geriatrics, primary care, palliative care, and/or long-term care. Ideal candidate will have demonstrated the ability to function well in a team as well as independently. Previous telehealth experience, use of electronic medical records, and comfort with mobile technology are also desired.
