Evaluate patients to develop individualized care plans and deliver speech therapy using specialized techniques in home settings. Collaborate with physicians and care teams to ensure integrated, patient-centered treatment and maintain accurate clinical documentation.
Requires a Master's or doctoral degree in Speech-Language Pathology and an active Californian SLP license. Candidates must have at least one year of clinical experience, CPR certification, and a valid driver's license with a reliable vehicle.
Evaluate patients and develop individualized care plans to address speech and language deficits. Deliver speech therapy using specialized techniques and equipment, and recommend communication devices when needed. Partner with referring physicians and the care team to ensure integrated, patient-centered treatment. Educate patients, families, and caregivers on treatment plans and goals. Participate in case conferences and care coordination meetings. Maintain accurate, timely clinical documentation per organization policy. Stay current in speech-language pathology practice and theory. Perform other duties as required for patient services. Master's or doctoral degree in Speech-Language Pathology. Active Californian SLP license. Minimum 1 year of clinical SLP experience. Current CPR certification with AHA. Valid driver's license and insured, reliable vehicle. Strong communication and organizational skills.
