Develops and implements age-specific plans of care for hospice patients through assessment, diagnosis, and evaluation. Collaborates with an interdisciplinary team to coordinate treatments and supervises home health aides to ensure quality patient care.
Requires a nursing degree and a current New York State Registered Professional Nurse license. Candidates should have at least 2 years of nursing experience, with a strong preference for acute care, hospice, or home care backgrounds.
Performs an age specific plan of care for a designated group of patients using the nursing process of assessment, diagnosis, outcome identification, planning, implementation and evaluation of patient care. Collaborates with physicians and other health team members in coordinating and implementing procedures and treatments. Uses leadership skills/clinical judgment in coordinating patient care and directing/delegating activities of the patient care unit team.
