The Case Manager is responsible for securing permanent housing for formerly homeless individuals and providing short-term service planning. They coordinate community-building activities and maintain relationships with local agencies to provide essential social services.
Candidates must have an Associate degree in Social Work or a related field with at least 3 years of experience working with homeless populations. A valid Washington State driver's license, reliable transportation, and proficiency in MS Office/Google Suite are required.

The Case Manager is responsible for securing permanent housing for people who were formerly experiencing homelessness, short-term service planning, maintaining relationships with community agencies and service providers, brief intervention and crisis response, information and referral assistance, and coordination of community-building activities.
