The Physical Therapist plans and implements individualized treatment programs for patients of all ages in a home care setting. Responsibilities include regularly re-evaluating care plans and supervising Physical Therapist Assistants according to state guidelines.
Candidates must hold a degree from an APTA accredited program and be licensed as a Physical Therapist in New Hampshire. BLS, CPR certifications, a valid driver's license, and proof of automobile insurance are also required.

About the Job: Provides professional physical therapy services to clients/patients of all ages and families whose needs change from basic to complex and are rapidly changing. Plans and implements age-appropriate specific treatment programs for individual client/patients with a level of skill in accordance with accepted principles and practice. What You’ll Do: Plans, develops, and implements physical therapy care plan appropriately. Regularly re-evaluates and updates appropriateness of care plan and physicians’ orders. Communicates appropriate information to patients, physicians, peers, managers, physicians, departments, and other agencies involved. Integrates problem solving methodology and quality improvement in resolving problems related to patient care and organizational processes. Orients and supervises PTA’s according to State of NH guidelines and VNA policy. Who You Are: Education: Degree from an American Physical Therapy Association (APTA) accredited program required.
Participate in rotating weekend commitment.
