Provides professional nursing services to home-based patients through assessment, planning, and evaluation. Manages primary care functions and independently administers prescribed treatments and medications.
Requires a nursing diploma or associate degree, with a BSN preferred, and a valid RN license. Candidates must have at least one year of experience with the same patient population and a valid driver's license.

General Summary Provides professional nursing services to individuals in the home through a process of nursing assessment, planning, care, and evaluation in accordance with physicians' orders, the established plan of care, the patient's diagnosis, corporate policy and procedures and the Nurse Practice Act. Carries primary care management functions for assigned patients. Safely and independently administers prescribed treatments, medications, and nursing interventions based on assessment and plan of care. May require specialized skills depending on VNA assignment.
