Provide personalized one-on-one care to patients in their homes, assisting with activities of daily living and mobility. Maintain detailed records of patient progress and coordinate observations with supervisors.
Candidates must have experience with amputees, disabilities, and the use of a Hoyer lift. A personal vehicle and strong communication skills are required.

You will be directly working with patients, following a one-on-one care plan in patient homes. You operate with a big heart, not only providing personalized and attentive care, but by building genuine relationships with those you serve. You are detail-oriented, highly organized, and committed to creating a healthy environment that meets patient needs. Strong verbal and written communication skills are needed to succeed in this role.
Must have experience working with people with Disabilities/Amputees & Hoyer Lift. Must have a car. Maintain records of patient care, conditions, progress or problems to report and discuss observations with supervisor
Assist client with full ADLs (Activities of daily living) Assist with cooking, medication reminders, extensive transfer support (bed, chair, mobility transitions/ if needed) Assist with ambulation (wheelchair support) Skilled services (wound care) If Needed Hoyer Lift exp. Required
