The Care Coordinator performs a variety of casework duties and provides case management services to members in nursing facilities. This includes assisting adult patients and their families with personal and environmental difficulties associated with medical conditions.
A Bachelor’s Degree in Health & Human Services or a related field is required, along with at least one year of experience in long-term care and health plans. Preferred qualifications include nursing home experience and case management or care coordination experience.
