Together, we improve the health of patients and their families, making our communities – and the world – stronger We are seeking a Social Worker for our Hudson Valley Medical Group to provide behavioral health services within our primary care sites as part of the collaborative care model.
• The
• Social
• Worker will be responsible for the following duties: Completes psychosocial assessments, appropriate behavioral health and disease-specific screeners, as well as other clinical duties within the organization's service standards and in an independent manner with appropriate use of supervision Provides evidence-based behavioral health interventions and psychoeducation. Assesses and systematically collects and tracks data in a patient registry related to patient behavioral health status and works with the care team to develop the plan of care. Participates in-person or virtual huddles when shared patients are discussed. Utilizes data, including registries, to continuously meet care coordination needs across settings in conjunction with the patient, family and/or other care providers. Develops and maintains a working knowledge of community resources and managed care review processes. Assumes responsibility and accountability for effectively managing behavioral health care of individual patients. Documents history, assessments and interventions to reflect comprehensive and integrated approach to patient care. Ensures complete, accurate and timely documentation in the electronic medical record. Facilitates support groups with the goal to ensure patients receive adequate psychoeducation on their illness, as well as effective strategies for coping Identifies learning needs of the patient and families, provides patient education; educates by adapting to patient and family learning style utilizing the teach-back method. Attends team meetings and case conferences to present and discuss patient disposition and outcomes data. Assists with the coordination of care across the continuum (e.g. inpatient, outpatient, home, skilled nursing facility, schools) serving as a key person to connect patients with resources in the community ensuring patients receive appropriate services via referrals and linkage Assists with management of urgent and emergent patient care situations. Assists with the management of patients with multiple chronic health conditions, acute episodes of care, behavioral health challenges while promoting and supporting prevention and wellness. Provides care that is safe, ethical, patient- and family-centered, culturally congruent and evidence-based. Performs other related duties as assigned and as appropriate to scope, education, and/or training. When qualified, provides supervision to graduate social work student interns (SIFI eligible) Experience a place where Social Workers are respected. Preferred Criteria

At least three years of social work experience New York State license as a Licensed Clinical Social Worker (LCSW) SIFI certification preferred Experience working with individuals and families affected by mental illness. Experience providing evidence-based psychotherapeutic approaches.

Bilingual
