Key Responsibilities
Perform initial and ongoing patient assessments, including OASIS evaluation and documentation.
Develop and implement tailored treatment plans in consultation with physicians and clinicians, adjusting as needed.
Collaborate with the interdisciplinary care team to optimize patient outcomes.
Provide education and support to patients and families/caregivers on therapy programs.
Maintain accurate clinical records and report any changes in the patient’s condition to the physician.
Supervise Physical Therapy Assistants in accordance with agency policies and state regulations.
Participate in in-services, case conferences, and meetings to promote coordinated care with healthcare team members.
Identify patient and family/caregiver needs for additional services and make appropriate referrals.
Confirm daily visit schedules to ensure seamless coordination with other staff.
Communicate patient progress and changes with physicians and care teams.
Document all visits and progress notes on the same day of service.

Must be a graduate of an accredited physical therapy program with a current state license and BLS certification. Requires at least one year of PT experience and a valid driver's license with a reliable vehicle.
