Deliver skilled nursing care and create individualized care plans for patients in their homes. Provide medication administration, family education, and support for patient recovery and independence.
Must be a Registered Nurse licensed in Connecticut with at least one year of professional acute care experience. A BSN is preferred, and a valid driver's license with reliable transportation is required.
Graduate of an approved nursing education program, BSN preferred. Licensure as a Registered Nurse in the State of Connecticut.
