About the role

Hospice LPNs will provide hands-on care to patients nearing the end of life according to state nursing rules, following the established care plan after patient assessment. They will also teach and support patients and families regarding the illness and care plan, and help coordinate care by communicating with the hospice team.

  • You will: Give hands-on care to patients nearing the end of life, following state nursing rules. Follow the care plan after assessing the patient’s needs. Keep clear and timely records of the care provided. Teach and support patients and their families about the illness and care plan. Help coordinate care by communicating with the hospice team (like aides, chaplains, social workers). Manage pain and symptoms, checking how well treatments are working. Take part in training and quality improvement activities to grow professionally and improve care.

Qualifications

Graduate of an accredited practical nurse or vocational nursing program. Three years nursing experience. Community health/hospice or medical/surgical experience is preferred. Currently licensed as an LPN/LVN in the State. Comply with accepted professional standards and practice. Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order. Possess and maintain current CPR certification. Understand philosophy of hospice concept. Also understand needs of the terminally ill. Use own knowledge and experience base and other resources as necessary to make logical decisions and solve problems. Demonstrate good verbal and written communication, and organization skills. Requirements

Candidates must be a graduate of an accredited practical nurse or vocational nursing program and possess three years of nursing experience, with community health/hospice or medical/surgical experience preferred. Applicants must hold a current LPN/LVN license in the state, have a valid driver's license with an insured vehicle, maintain CPR certification, and understand the hospice philosophy and needs of the terminally ill.
