The specialist assesses the nutrition needs of high-risk patients and provides clinical counseling to manage chronic diseases and healthy eating habits. They focus on reducing avoidable hospitalizations and emergency room visits by addressing social determinants of health and food insecurity.
The Population Health Nutrition Specialist works collaboratively with other programmatic and clinical specialists assigned to respond to new value based payment models supported by Hawaii Managed Care Organizations.
The nutrition specialist assesses nutrition and diet health needs of patients at high risk of chronic disease and provides clinical counseling on nutrition issues and healthy eating habits in a culturally sensitive manner.
The nutrition specialist addresses gaps in care in patients identified as benefitting from clinical counseling related to food, nutrition and diet concerns while being mindful of social determinants of health, specifically food insecurity and food access.
The position requires knowledge of preventive and disease intervention strategies related to patients with chronic disease as well as those with co-morbid conditions including social and behavioral influences.
The position contributes to an overall strategy of addressing preventable costs of care, including reducing low acuity emergency room visits and avoidable hospitalizations by focusing on health education, disease prevention and primary care in a culturally sensitive, supportive healthcare home environment.
1. Registered Dietitian (with current CDR Registration from the American Dietetic Association) or RD eligible required
2. Licensed Dietitian in the State of Hawaii required
3. Master’s Degree in Nutrition (or coursework supplemented by public health courses, from an accredited college or university) preferred
4. Telehealth experience preferred
5. Experience in health services for chronically ill patients desirable
word processing, entering data onto and sorting data in spreadsheets, navigating through the internet, email, and experience with electronic health records
7. Communication skills (verbal and written) required
8. Ability to work in partnership with community health organizations
9. Ability to communicate effectively with staff members, community members, and patients
10.OSHA training annually
11.BCLS every two years
12.Hawaii Driver’s License strongly desired
13.Ability to work flexible hours required
Candidates must be a Registered Dietitian with a current CDR Registration and a valid license in the State of Hawaii. A Master's degree in Nutrition and experience with telehealth or chronically ill patients are preferred.
