The Family Resource Coordinator evaluates participant needs and refers clients to diverse community resources and social service agencies. They coordinate appointments and assist families in communicating with medical offices and DCF to increase their ability to self-advocate.
Requires an Associate's degree and five years of experience in health or social services, or an equivalent combination of education and experience. Candidates must obtain a Community Health Worker Certification within six months of hire and possess a valid driver's license.
The Family Resource Coordinator will work under the supervision of the Director of Alliance for Family Support in collaboration with Home Visitors & Clinical Supervisors working for subcontracting agencies. The Family Resource Coordinator is a community based position whose responsibilities include but are not limited to, working alongside Home Visitors evaluating needs of participants, referring clients to community resources, coordinating appointments, providing help with enrollments with diverse resources in the community. The Family Resource Coordinator assists families in their communications with medical offices, social service agencies, DCF and other organizations. All work with families is short term with the objective of increasing their ability to self-advocate.
