Responsibilities
- Patient Assessment & Triage
- Evaluate incoming patient phone calls to determine the urgency of cardiac-related symptoms and appropriate next steps.
- Provide telephone triage for symptoms such as chest pain, shortness of breath, palpitations, dizziness, edema, or blood pressure concerns, and guide patients to the appropriate level of care or follow-up. Care Coordination
- Assist in coordinating urgent and emergent care for patients with acute or worsening cardiovascular conditions (e.g., chest pain, heart failure exacerbation, arrhythmias, syncope).
- Communicate efficiently with cardiologists and clinical team members to ensure timely and effective care. Patient & Family Education
- Educate patients and caregivers on cardiovascular conditions, treatment plans, medications, and lifestyle modifications.
- Offer ongoing support and resources tailored to each patient’s needs, including symptom monitoring and when to seek urgent care. Clinical Documentation & Follow-Up
- Accurately document all patient interactions, assessments, and care plans using the electronic health record (EHR).
- Track patient outcomes and coordinate follow-up care, diagnostic testing, and referrals to ensure continuity of care.
Qualifications
Licensure: Current Registered Nurse (RN) license in Iowa Experience: All levels of RN experience will be considered. Prior experience in neurology or specialty care is a plus, but not required.
Education
Associate Degree in Nursing (ADN) required; Bachelor of Science in Nursing (BSN) preferred. For positions within UnityPoint Clinic, team members may be required to work at or to float to other locations to support optimized staffing
