The Nurse Practitioner performs routine and urgent assessments, including Medicare-required Face-to-Face visits, to determine hospice eligibility and provide care coordination. They are responsible for prescribing medications, ordering tests, and collaborating with an interdisciplinary team to manage terminally ill patients.
Candidates must be an unrestricted Nurse Practitioner in Nevada with active PECOS enrollment for hospice. Experience with Medicare-certified home health or hospice agencies and the ability to travel for home visits are required.
