Manage a caseload of clients by monitoring progress toward service plan goals and coordinating access to mental health, recovery, and primary care services. Facilitate group sessions and maintain accurate documentation in client tracking databases while providing crisis intervention.
A high school diploma is required, though a bachelor's degree in human services or a related field is preferred. Candidates must have at least 2 years of experience in human services or case management.

The Doe Fund is currently seeking a high-energy, self-motivated, experienced Case Manager to join our team. The successful candidate will have experience working with formerly incarcerated individuals, Mental Health Organizations, Homeless Services, and/or Housing organizations. This position will be based out of our Harlem Center for Opportunity in Manhattan.
