Acts is currently seeking qualified candidates for our Home Health Physical Therapist position.

In-house program within a welcoming, slower-paced community environment that delivers holistic care with a strong focus on prevention Specialized on-site training delivered during paid work hours, some sessions eligible for CEUs State-of-the-art rehab gyms and equipment Opportunity to build relationships by following residents across the full continuum of care, including inpatient, home health, outpatient, and fitness services Clinically driven, patient-centered treatment decisions guide all programs and care plans Cross training provided to support all rehabilitation areas within our communities Limited travel within and between our private communities Due to our company being a non-profit organization, student loan forgiveness is available after 10 years!

In this role, you will be responsible for providing physical therapy services to home health residents/clients in their home environment to promote functional independence, safety, and quality of life.

Evaluates residents/clients, develops individualized plans of care, implements evidence-based interventions, and collaborates with the interdisciplinary care team to achieve optimal outcomes in compliance with physician orders, state practice acts, and federal and state home health regulations.

Requirements

The ideal candidate will meet the following requirements: Master’s degree or Doctoral degree in Physical Therapy from an accredited program Minimum of 1-2 years of experience in home health is preferred Valid Physical Therapist license or eligibility for license in the state(s) where services are rendered Team members are eligible for a generous benefit package including health benefits (medical, prescription, dental and vision), flexible spending accounts, life insurance, disability programs, 401(k) plan (with 4% company match after one year of employment), paid time off and holidays, and much more!
