The Case Manager serves as a guide and advocate for justice-involved individuals, facilitating individual and group sessions to support reentry goals. They collaborate with a multi-disciplinary care team to monitor progress and connect participants with essential community resources.
Requires a Bachelor's degree in a social service-related field and 1-3 years of experience in case management with vulnerable populations. Candidates should be familiar with 12-step programming and trauma-informed care practices.
Description Concordance’s reentry program is designed to dramatically reduce recidivism by helping individuals with justice involvement lead meaningful, abundant, and joyful lives. Our holistic, evidence-based model offers a wide range of services - therapy, case management, yoga, behavioral health and substance use recovery groups, spiritual care, life skills, medical care, education, and job readiness. Each participant is formally supported by our Care Team for a full year, ensuring stability and long-term success. Now operating in Missouri, South Carolina, and Virginia, Concordance is a national leader in compassionate, innovative reentry care.
The Case Manager serves as a trusted guide, advocate, and accountability partner for participants by monitoring progress and helping participants navigate through obstacles. Case Managers facilitate both individual and group sessions with participants and serve as the designated leader of participants’ care teams. The Case Manager collaborates with the care team to ensure participants are obtaining quality support as they advance through the program model.
