The Hospice LVN ensures the quality and safe delivery of hospice services by providing direct hands-on care. They are responsible for obtaining orders to promote patient comfort and keeping the Case Manager updated on condition changes.
Candidates must hold a current LVN license in the State of California and have at least one year of acute-care experience. Experience in hospice and proficiency with EMR systems are preferred.

The Hospice LVN position is open in our Rancho Cucamonga location and surrounding areas. This is a Full-time position during the day shift from 8:00am-4:30pm.
of the Hospice LVN: · Current LVN license in State of CA · Minimum one year of acute-care experience · Experience in hospice preferred · EMR experience preferred
· Ensures quality and safe delivery of Hospice services · Implements current nursing practices and provide direct hands-on care within scope of practice · Obtains orders as needed to promote comfort and quality of life · Keep Case Manager informed of all changes in condition and orders obtained
