Manage a patient caseload by coordinating care plans, conducting assessments, and supporting treatment adherence for patients with complex needs. Coordinate referrals to community resources and monitor medical data to improve overall quality of life.
Requires an active California LVN or RN license, BLS certification, and completion of an approved IV therapy course. Candidates must be able to travel between sites and have working knowledge of Electronic Medical Records.
We are seeking a compassionate, organized, and patient-focused Float Specialty Nurse Case Manager to provide advocacy, care coordination, and guidance for patients with complex medical and social needs at our Dora Street and Lakeview Health Centers. Working collaboratively with multidisciplinary care teams, the Nurse Case Manager ensures patients receive high-quality, comprehensive care while empowering them to make informed healthcare decisions and improve their overall quality of life.

Active California LVN or RN license Completion of an approved IV therapy course Current BLS (Basic Life Support) certification Willing and able to travel between sites Working knowledge of Electronic Medical Records use and process
Preferred Qualifications: Bilingual, English/Spanish, fluent Previous experience working as an LVN or RN in an outpatient setting
