Coordinate client-centered services for Ryan White clients to improve access to healthcare and psychosocial support. Conduct non-medical assessments and develop individualized service plans to ensure continuity of care.
Requires at least 2 years of experience in healthcare delivery, care navigation, or case management, specifically working with diverse and LGBTQIA+ populations. A Bachelor's in Social Work is preferred along with experience in HIV programming.
2+ years of experience in a healthcare delivery system including care navigation, advocacy, and/or case management 2+ years of experience in working with diverse populations, including LGBTQIA+ and communities of color Demonstrated experience taking initiative and to exercising independent judgment, decision-making, and problem-solving expertise Proficient using computer software applications, such as Windows and Microsoft Strong written and verbal communication abilities
Bachelor's in Social Work from a school accredited by the Counsel of Social Work Education 2+ years of direct experience with HIV counseling and testing, case management and/or HIV programming Familiarity and experience using a trauma-informed approach to working with individuals Experience and comfort in issues related to substance use, harm reduction, homelessness, mental health, and LGBTQIA+ communities Ability to work harmoniously with diverse groups of individuals Experience working with an ethnically, culturally and racially diverse work staff Ability to deal effectively with stress relating to the changing organizational, personnel, and healthcare environment Compensation Based on job duties and requirements, this position is placed in salary band 40 with an hourly rate starting at $20.80 per hour non-exempt. Initial placemen will be based on hired candidate's qualifications and relevant experience.
