Resource Navigator (DAYTIME POSITION)

• 4. Advocacy & Barrier Removal
• Advocate on behalf of participants with service providers and partner agencies
• Identify systemic barriers and escalate issues to program leadership
• Support participants in navigating complex systems such as healthcare, workforce programs, and public assistance

• 5. Collaboration & Partnerships
• Develop and maintain relationships with community organizations, employers, and service providers
• Participate in cross-agency meetings and case conferencing
• Support outreach and engagement efforts to expand service access

• 6. Data Tracking & Reporting
• Track participant outcomes, service utilization, and referral success rates
• Prepare reports for internal and external stakeholders
• Ensure accuracy and confidentiality of participant records

Qualifications

• Previous experience as a Care Coordinator or in a similar position is preferred
• Strong problem-solving and organizational skills
• Ability to manage multiple projects or tasks and prioritize appropriately
• Ability to work in fast-paced situations and make sound decisions quickly
• Excellent interpersonal skills and high level of compassion
• Strong verbal and written communication skills

Compensation

$30.00 - $35.00 per hour
