We are seeking a Full-Time Physical Therapist (PT) to join our dynamic rehabilitation team and help residents regain strength, mobility, and independence.

Physical Therapist Position Overview: Physical Therapy team members assist with ensuring the health and well-being of individuals by providing comprehensive and quality therapy services.

This position is responsible for administering skilled physical therapy treatments ensuring that the highest degree of quality care is maintained at all times.

State specific Physical Therapy license is required.

Responsibilities

Evaluate and develop individualized treatment plans for residents Deliver skilled physical therapy services in accordance with physician orders Collaborate with interdisciplinary team members to optimize care plans Monitor, document, and adjust treatments based on resident progress Educate residents and families on therapy goals and outcomes

Qualifications

Doctorate or Master’s degree in Physical Therapy from an accredited program Current Vermont Physical Therapy license (or eligibility) Strong clinical, communication, and documentation skills Passion for working with geriatric and post-acute populations

Responsibilities

Physical Therapist: Initiates skilled physical therapy treatments in accordance with physician’s orders; maintains required contact with physician. Maintains written evaluations, progress notes and discharge summaries for individuals in physical therapy program. Confers with other health care staff regarding special needs of individuals to enhance rehabilitation or maintenance of function. Provides input for meetings and attends during absences of the Director Physical Therapy. Maintains statistical records of individuals’ treatments. Assists with in-service training programs upon request. Records and updates individual’s plan of care in the Inter-disciplinary Kardex. Submits requisition for supplies needed in the treatment to Director Physical Therapy. Assumes all duties of the Director Physical Therapy during absences including supervision of Physical Therapy Aides and responsibility for all necessary documentation. From Up to

Qualifications

Physical Therapist: Graduation from an approved school of physical therapy required. State license to practice physical therapy required. Two years’ experience in physical therapy preferred. Ability to direct others necessary. This position requires regular interaction with residents, coworkers, visitors, and/or supervisors. In order to ensure a safe work environment for residents, coworkers, visitors, and/or supervisors of the Company, and to permit unfettered communication between the employee and those residents, coworkers, visitors, and supervisors, this position requires that the employee be able to read, write, speak, and understand the English language at an intermediate or more advanced level. Requirements
