The Navigator provides case management and direct outreach to individuals with mental health or co-occurring disorders at high risk for treatment admission. Key duties include developing support plans and acting as a liaison between members, families, and community agencies.
Candidates must have a high school diploma or GED, a valid driver's license, and reliable transportation. A minimum of 2 years of experience or lived experience working with underserved populations, homelessness, or serious mental illness is required.
High School Diploma, GED or equivalent, required. Valid Driver’s License with acceptable driving history and reliable, insured transportation. Minimum of least 2 years’ experience working with or lived experience with homelessness, justice involvement, adults with serious and persistent mental illness, substance use disorders, and underserved populations, required. Excellent communication, documentation, time management and organizational skills required.
