The Children’s Residential Licensed Practical Nurse (LPN) supports the physical, emotional, and medical wellbeing of each youth in the residential programs of the agency, by ensuring timely and appropriate health care. The LPN focuses on promotion of health through advocacy and education to youth on healthy lifestyle options and treatment compliance. The LPN works in a care-team environment with other residential staff utilizing in-depth consideration of trauma informed care to ensure youth successfully transition through treatment towards a lower-level of care.
Responds to all onsite healthcare needs of children, including illnesses, first aid, PRN medications, and overall, wellbeing issues of each child. Assists with, track and schedule physician appointments along with documentation for supportive transportation. Assists with and ensure that all children have up to date medical exams, immunizations, and healthcare in compliance with state and DCFS regulations. Ensures that medical information for each child in the digital and paper files is accurate and up to date, as well as compliant with DCFS standards of care. Is trained and informed on all legal parameters for consents necessary and obtains consents for medications and procedures in compliance with DCFS and state regulatory procedures. Assists with training staff in regard to the health care needs of children including but not limited to medications and adverse reactions. Trains staff with all needed medication and healthcare procedures for the residential program. Participates in staff meetings, Child and Family Team meetings, and other leadership and management team meetings in order to provide medical insights and information for medications and medical needs for residents. Participates in new admission referral review to assist with determination of level of care and acuity of referral as it relates to supporting medical care for a resident. Ensures continuity of care among nursing team by documenting and communicating actions, incidents and patient needs. Perform additional duties as assigned. Minimum of one year of experience working with youth. Equivalent combinations of education and experience may be considered.
Licenses/Certifications Required: Licensed LPN in the State of Illinois
Manager-Specific: Ability to read, analyze, and interpret common scientific and technical journals, financial reports, and legal documents.
Communicate effectively in writing as appropriate for the needs of the audience. Effective communication, documentation, and ability to apply medical knowledge in a residential care setting.
