The Occupational Therapist evaluates patients' functional status and develops individualized treatment plans to improve independence and quality of life at home. Responsibilities include providing therapeutic interventions, educating caregivers, and collaborating with an interdisciplinary team to coordinate patient-centered care.
Candidates must be graduates of an accredited Occupational Therapy program with current NBCOT registration and state licensure. A valid driver's license, CPR certification, and preferably one year of clinical experience are required.

What You’ll Do As a Home Health Occupational Therapist, you will play a critical role in helping patients regain independence, improve daily function, and safely navigate life at home. You’ll partner closely with patients, families, and interdisciplinary teams to deliver personalized, high-impact care. Patient Care & Clinical Impact Evaluate patients’ functional status, including mobility, strength, coordination, communication, and ability to perform daily activities Assess home environments to identify safety risks and recommend modifications or adaptive equipment Develop individualized treatment plans with clear, measurable goals focused on improving independence and quality of life Provide therapeutic interventions to restore function, including retraining in daily living activities Fabricate splints and train patients in the use of adaptive, prosthetic, and orthotic devices as appropriate Support patients returning to work through functional and work capacity evaluations Educate patients, families, and caregivers on treatment plans, safety, and ongoing care needs Collaborate with the care team to coordinate services and ensure seamless, patient-centered care Participate in discharge planning to support successful transitions and continued progress Care Coordination & Communication Document patient care accurately and timely within the electronic medical record (EMR) Communicate plans of care with physicians and interdisciplinary team members Participate in case conferences, team meetings, and ongoing clinical collaboration Maintain strong, professional relationships with patients, families, and care team members Leadership & Oversight Supervise Certified Occupational Therapy Assistants (COTAs) in accordance with state regulations and company policy When therapy is the sole skilled service, provide oversight of home health aide care as required What Sets You Apart Strong clinical judgment with a patient-first mindset Ability to build trust and rapport with patients and families in a home setting Excellent communication, organization, and time management skills A proactive, compassionate approach aligned with our core values: trustworthy, capable, compassionate, proactive, and called
Graduate of an accredited Occupational Therapy program Current registration with the National Board for Certification in Occupational Therapy (NBCOT) Current state licensure as an Occupational Therapist Minimum of one (1) year of clinical experience preferred (home health experience a plus) Current CPR certification Valid driver’s license, reliable transportation, and auto insurance
