Conduct person-centered assessments and develop Individual Support Plans to help adults with disabilities and the aging population access necessary services. Coordinate with medical professionals and community resources to ensure participant safety, satisfaction, and independence.
Requires a Bachelor's degree in Social Work, Psychology, Human Services, or a related field. Candidates must have a valid driver's license, reliable transportation, and the ability to travel up to 50% of the time.

Develop, implement, and regularly update Individual Support Plans (ISP) to ensure participants receive the right services. Provide participants with choice and advocacy, helping them make informed decisions about providers and care options. Coordinate with medical professionals, behavioral specialists, family members, and community resources to deliver seamless, high-quality care. Monitor and evaluate services to ensure participants' safety, satisfaction, and progress toward independence. Support both individuals with disabilities and members of the aging population in accessing the community resources they need. Maintain documentation, confidentiality, and compliance with all program and ethical standards. Participate in ongoing training and professional development to strengthen your skills and expand your impact. What You Bring Bachelor's degree in Social Work, Psychology, Human Services, or related field. Strong organizational, communication, and advocacy skills. Comfort working independently and traveling (up to 50%) to meet with participants in their homes and communities. Valid driver's license, auto insurance, and reliable transportation. Commitment to maintaining required certifications and state clearances.
