• What you’ll do
• Deliver 1:1 PT care in patient homes Build clear, goal-based treatment plans Improve mobility, strength, and independence Coordinate with RN, OT, and care teams Complete notes during paid work hours Manage 5–6 visits daily Adjust care plans based on patient progress Must-haves Active Missouri Physical Therapist license DPT or equivalent Physical Therapy degree Valid driver’s license and reliable vehicle Ability to work independently in the field Nice to have Home health experience OASIS documentation exposure Medicare patient experience Perks &
