Provide individualized community services, advocacy, and crisis support to individuals recovering from mental health or substance abuse issues. Collaborate with the ACT team to conduct clinical assessments and implement person-centered treatment plans.
Requires a high school diploma or GED and at least five years of lived experience with serious mental illness or substance addiction. Candidates must be in stable recovery and demonstrate at least one year without crisis or relapse.
Participates with other team members in conducting comprehensive clinical assessments, person centered plans, crisis consultations and other primarily QP functions so as to provide essential expertise and consultation to the entire team to promote a culture in which each person’s point of view and preferences are recognized. Provides ongoing assertive outreach and supportive services to address the persons served needs effectively. Provides interventions to offer support and guidance in all areas of functional domains: adaptive, communication, personal care, domestic, psychosocial, problem solving, etc in preventing, overcoming or managing the person’s level of functioning and enhancing his/her ability to remain in the community. Participates in daily interdisciplinary treatment planning, daily staff meetings and review of persons served and functional needs Completes all documentation as required by procedures and statute. Participates in regular supervision with the ACT Team Leader to enhance all competencies in this position. All other duties as assigned by supervisor Units Billed Minimum Requirement: Along with other team members, is responsible to ensure adequate service delivery is provided to all persons served in accordance with National Program Standards, state service definition requirements, and the needs of the persons served. Education/Experience: Peer Support services are delivered by staff who have the life experience of being diagnosed with a serious mental illness or substance addiction problem, and are currently in recovery, stable, and living without relapses. The Peer Support Paraprofessional has a high school diploma, or GED and at least five years of life experience with a serious mental illness or substance addiction. The paraprofessional also self identifies as an individual with a psychiatric or addictions disability. Paraprofessional staff who work no more than 10 hours a week providing Peer Support and have not completed their 20 hours of training have two years to complete the training required. The Peer Specialist also demonstrates at least one full year in his/her own life without crisis, relapse, or psychiatric or substance abuse inpatient treatment, or criminal justice involvement and may not be a family member or related to the consumer. Peer Support Specialists are supervised by a Qualified Professional. The Qualified Professional Peer Support meets same requirements as listed above; but also, meets QP status based on education of Bachelor’s degree and required experience to meet QP standards.
