Southwestern Palliative Care and Hospice

Conduct in-home visits and physical assessments to develop and manage individualized care plans for hospice patients. Prescribe treatments for symptom control and provide education and support to families during end-of-life care.
Requires a Master's degree from an accredited Nurse Practitioner program, current state licensure, and CPR certification. Candidates must have at least one year of NP experience and one year of experience in hospice or palliative care.
