Deliver physical therapy interventions and assist patients with exercises, mobility, and rehabilitation as directed by a supervising Physical Therapist. Document all services in the EMR and ensure compliance with HIPAA and CMS regulations while traveling to patient homes.
Must be a graduate of an accredited PTA program with a current, unrestricted New York State license and CPR certification. A valid driver's license and reliable transportation are required.
Position: Physical Therapist Assistants We are looking for caring and motivated Physical Therapist Assistants (PTAs) to join our team in all five Boroughs. Whether you’re an experienced PTA or just starting your career, we are here to support you.
Deliver physical therapy interventions as directed by the supervising Physical Therapist and outlined in the Plan of Care. Assist patients with exercises, mobility training, gait, balance, and other therapeutic activities to support rehabilitation and safety. Monitor patient response to treatment and communicate changes or concerns to the supervising PT. Instruct patients and caregivers in home exercises and safe use of assistive devices or techniques. Accurately and promptly document all services provided in the electronic medical record (EMR). Maintain ongoing communication with the PT to ensure coordinated, effective care. Ensure compliance with agency, state, and federal guidelines, including HIPAA and CMS regulations. Travel to patient homes/facilities within assigned geographic areas. Participate in interdisciplinary case conferences and supervisory support as needed. All duties as assigned

Required: Graduate of an accredited Physical Therapist Assistant program.
Current and unrestricted license to practice as a PTA in the State of New York.
Current CPR certification. Valid driver’s license and access to reliable transportation. Strong interpersonal, communication, and time management skills.
Experience in home health or rehabilitation settings.
