Perform initial home health assessments and physical histories for patients and their families. Develop and evaluate comprehensive care plans based on nursing diagnoses and OASIS scoring.
Requires an Associate degree in nursing and a valid New Hampshire RN license with at least one year of recent acute care experience. CPR certification and a satisfactory criminal and motor vehicle record are mandatory.
Registered Nurse (RN) Homecare Admissions opening full time 40 hrs. a week. This position is Monday - Friday 8:00am - 4:30pm with weekend / holiday rotations. Offering $12,500 sign on bonus!
About the Job
As a Registered Nurse in Homecare or Hospice, you’ll deliver skilled, compassionate nursing care directly in the patient’s home. You’ll complete comprehensive assessments—including OASIS—develop individualized care plans, and collaborate closely with physicians, families, and the interdisciplinary team to ensure high‑quality, goal‑directed, cost‑effective care. This role blends clinical expertise, autonomy, and relationship‑centered care to support patients in maintaining comfort, safety, and independence at home.
