Job Description

A Brief Overview Coordinates data collection for the Resident Assessment Instrument (RAI), including the MDS, CAAS and the Multidisciplinary Care Plan for long term care. What you will do Completion of the MDS, CAAS and Care Plans are done by required completion dates and submit to the state of Iowa by submission date. Evaluate nursing and clinical intervention outcomes, conferring with other health care team members as necessary. Monitor resident medication usage and results. Participates in discharge planning and other necessary team conferences. Interview resident as designated and review documentation related to medical conditions, such as adverse reactions to medication or treatment, condition change, or progress or lack of progress with interventions. Work as part of a health care team to assess resident needs, plan and modify care, and implement interventions as appropriate. Demonstrates accurate keyboarding proficiency and computer navigation skills. Participate in activities aimed at professional growth and development including conferences or continuing education activities. Collaborates with assigned RN to refer residents requiring more specialized or complex treatment to psychiatrists, primary care physicians, or other medical specialists. Must have current knowledge of the entire RAI process and current regulatory requirements related to this process and implement. Qualifications Associate’s Degree (AA) In nursing (ADN) Required Bachelor’s Degree in Arts/Sciences (BA/BS) In nursing (BSN) Preferred 1-3 years Nursing home facility experience within the last three years Required 1-3 years Experience in the RAI process Preferred 1-3 years Experience with Microsoft Suite (Word, Excel, PowerPoint, Teams, and Outlook) Preferred Knowledge of principles and processes for providing customer and personal services. This includes customer needs assessment, meeting quality standards for services, and evaluation of customer satisfaction. Knowledge of the structure and content of the English language including the meaning and spelling of words, rules of composition, and grammar. Knowledge of human behavior and performance; individual differences in ability, personality, and interests; learning and motivation. Being aware of others' reactions and understanding why they react as they do. Actively looking for ways to help people. Giving full attention to what other people are saying, taking time to understand the points being made, asking questions as appropriate, and not interrupting at inappropriate times.
