Provide psychosocial assessments, counseling, and crisis intervention to hospice patients and their families. Collaborate as part of an interdisciplinary team to coordinate patient care with the Case Manager.
Requires an MSW from an accredited school and a Certified Social Worker (CSW) credential. Candidates must have one year of paid social work experience in a hospice, medical, or clinical setting post-MSW.
