Hospice Savannah Inc. - SAVANNAH, GA 31406Job Summary The PACE Nurse Practitioner delivers direct primary medical care to PACE participants, conducting comprehensive initial and ongoing assessments as an integral member of the Interdisciplinary Team care planning process. The NP manages chronic and acute health conditions under the supervision of the Medical Director or Primary Care Physician, initiates evidence-based interventions, and provides primary care education to participants, caregivers, and staff to promote optimal health outcomes. QualificationsMinimum Qualifications Bachelor's degree in nursing and Master's degree in advanced clinical practice or graduate of post Master's certificate program for nurse practitioner (CMS required). Licensed as Registered Nurse (R.N.) in the State of Georgia. Certified as a nurse practitioner by a national certifying organization recognized by the State Board of Nursing(CMS required). One year of direct primary care experience with a frail, elderly, or long-term care population. Experience caring for frail older adults' physical, mental, emotional, and social needs. Demonstrates the ability to work in an interdisciplinary team setting effectively (CMS required). Demonstrates skills and knowledge as outlined in primary care provider competency requirements, including effective skills in physical assessment and chronic disease management for frail older adults. Be medically cleared for communicable diseases and have up-to-date immunizations and vaccines prior to engaging in participant care (CMS required). Have not been convicted of criminal offenses related to involvement in Medicaid, Medicare, other health insurance or health care programs, or social services programs under title XX of the Act (CMS required). Have not been convicted of criminal offenses pertaining to physical, sexual, drug, or alcohol abuse. Not excluded from participating in Medicare and Medicaid programs (CMS required). Competencies, Preferred Knowledge, Skills, and Abilities Certified as a Geriatric Nurse Practitioner (G.N.P.) with prescriptive authority 3-5 years nursing experience with Nurse Practitioner experience Experience working in a managed care environment, demonstrating the ability to successfully lead primary care provider staff and work effectively with hospitals, nursing homes, ancillary services providers, and physician specialists to coordinate and manage care for a medically complex population. Ability to educate participants, caregivers, and staff on disease management, preventive care, medication adherence, and self-care strategies Competence in conducting comprehensive initial and ongoing assessments, including history-taking, physical exams, and diagnostic interpretation. Demonstrated knowledge of regulatory requirements for PACE and Center of Medicare and Medical Assistance. Time management, organization, and prioritization in a fast-paced environment Requires basic computer skills in Microsoft Word products including Word and Outlook. As needed, support to Steward Center of Palliative Care Physical Demands & Work Environment Position requires moderate to intense visual concentration, reading, using a computer and reviewing patient charts. This position has frequent contact with patients, families, medical facilities, physicians, physician office staff, vendors/suppliers and HSI staff requiring excellent communications and problem-solving skills and professional demeanor. There is a risk of exposure to infectious diseases, which may require use of standard precautions. The NP requires the mobility to ambulate or lift a patient with or without the assistance of devices in excess of 75 pounds using proper body mechanics. Face-to-face recertification visits in hospital, long-term care facility or home environment with patients. Position Type and Expected Hours of Work Full Time- 40 hours per week Travel Must be able to travel as required

Requirements

Candidates must possess a Bachelor's degree in nursing and a Master's degree in advanced clinical practice or a post-Master's certificate, along with an active Registered Nurse license in Georgia and national Nurse Practitioner certification. A minimum of one year of direct primary care experience with frail, elderly, or long-term care populations is required, alongside demonstrated ability to work effectively within an interdisciplinary team.
