Locations
Conduct psychosocial assessments and provide clinical counseling to hospice patients and their families in a home setting. Collaborate with an interdisciplinary team to coordinate holistic care and maintain confidential documentation.
Requires a Master's Degree in Social Work and a current Washington State Social Work License. Prior experience in hospice or palliative care is strongly preferred.
