Position Title: Nurse Practitioner

Reports To: Senior Director of Compliance & Clinical Operations

Location: Onsite in our Tampa Office, Monday – Friday | 9:00 AM – 5:00 PM |

Job Summary

The Nurse Practitioner (NP) will play a crucial role in delivering comprehensive healthcare services in both telehealth and in-person environments. You will manage chronic and acute conditions, oversee patient monitoring through RPM, and collaborate with interdisciplinary teams to provide patient-centered care. This role also supports CCM and CHI initiatives, ensuring ongoing care for chronic disease management and improving community health outcomes.

Key Responsibilities

  • Patient
  • Assessment: Conduct comprehensive assessments of patients' health conditions, including physical examinations, medical histories, and diagnostic test results.
  • Diagnosis and
  • Treatment: Diagnose and treat common acute and chronic illnesses, injuries, infections, and other medical conditions. Develop treatment plans and prescribe medications as needed.
  • Preventive
  • Care: Provide preventive care and health maintenance services, including vaccinations, health screenings, and lifestyle counseling.
  • Health
  • Education: Educate patients and their families about health conditions, treatment options, medications, and self-care techniques.
  • Care
  • Coordination: Collaborate with the Medical Director, Operations Manager, Care Navigators, and Medical Assistants to streamline patient care.

Telehealth

Offer seamless virtual healthcare consultations, referrals, and follow-up care through our telehealth platform.

Record

Keeping: Maintain accurate and up-to-date patient records and documentation in compliance with healthcare regulations and standards.

Prescription

Management: Manage and monitor patients' medication regimens, including adjusting dosages and renewing prescriptions as needed.

Compliance

Ensure all documentation and patient care follow federal and state regulations, including HIPAA and telehealth guidelines.

Community

Health: Support CHI initiatives aimed at reducing healthcare disparities in underserved communities.

Follow-ups: Ensure that patients receive appropriate follow-up care, either personally or through coordinated care teams.

Requirements

Education: Hold a Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) degree from an accredited nurse practitioner program.

Licensure

  • Possess a valid and unrestricted nurse practitioner license in the

  • Certification: Be certified as a nurse practitioner by a recognized certifying body, such as the American Nurses Credentialing Center (ANCC) or the American Academy of Nurse Practitioners (AANP).

  • Skills: Demonstrate proficiency in conducting physical assessments, diagnosing medical conditions, and developing treatment plans.
  • Communication
  • Skills: Have excellent interpersonal and communication skills to effectively interact with patients, families, and healthcare team members. Bilingual in Spanish is required.
