The Nurse Practitioner will manage acute and chronic conditions through both telehealth and in-person care. Responsibilities include conducting patient assessments, developing treatment plans, and supporting community health initiatives.
Candidates must hold an MSN or DNP degree and possess a valid, unrestricted nurse practitioner license in Florida. Proficiency in Spanish and certification from a recognized body like ANCC or AANP are required.

Position Title: Nurse Practitioner
Reports To: Senior Director of Compliance & Clinical Operations
Location: Onsite in our Tampa Office, Monday – Friday | 9:00 AM – 5:00 PM |
The Nurse Practitioner (NP) will play a crucial role in delivering comprehensive healthcare services in both telehealth and in-person environments. You will manage chronic and acute conditions, oversee patient monitoring through RPM, and collaborate with interdisciplinary teams to provide patient-centered care. This role also supports CCM and CHI initiatives, ensuring ongoing care for chronic disease management and improving community health outcomes.
Education: Hold a Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) degree from an accredited nurse practitioner program.
