Responsible for managing resident care by collaborating with an interdisciplinary team and acting as a liaison between residents, families, and medical staff. Duties include conducting social work assessments, managing insurance and fiscal components, and coordinating care transitions and discharge planning.
Requires a Bachelor's or Master's degree in Social Work or a related field. Candidates must have at least one year of social work experience in a skilled nursing facility, including rehab and long-term care.
Per diem hours -
Responsible for the resident management process by collaborating daily with Interdisciplinary team (IDT) members involved in the care of residents including nursing staff, rehab, admissions, medical team and Administrator. Identifies areas needing attention/improvement, makes recommendations and directs interventions to facilitate change. Functions as resident liaison to IDT and families. As needed, provides social work services to Assisted Living and Independent Living residents on the McLean campus. Responsible to ensure Resident Rights are maintained at all times, especially during care transitions. Liaisons for all room changes within SNF and between SNF and AL/IL and team. JOB FUNCTIONS: Responsible for ensuring fiscal components of non-coverage, insurance, PASSR are completed accurately and timely. Issues notice of non-coverage to resident/responsible party and keeps IDT informed of any insurance related requests such as demand bills and medical reviews. Meets with all new admissions to complete Social Work assessment and monitors for adjustment. Facilitates family team meetings. Coordinates with resident/responsible party/family and members of IDT to facilitate care planning and goals upon admission and quarterly/as needed. Accountable for maintaining timely documentation including progress notes, care plan updates and MDS sections. Responsible for tracking and completing levels of care in ASCEND. Responsible to provide weekly updates to residents and/or responsible party in relation to discharge planning and/or care transitions. Maintains good working relationships with staff, residents, families, physicians, community and government agencies by establishing an environment of strong customer service. Identifies, develops and/or participates in Quality Improvement projects. Attends staff development and continuing education programs to ensure up to date knowledge of regulatory aspects of social work responsibilities. Participates in quarterly peer audits. Performs related work as assigned. WORKING CONDITIONS: Typical Physical Demands: Requires corrected vision and hearing within normal range. Requires the use of office equipment such as telephones, fax machines, computers, and copiers. Requires eye-hand coordination and manual dexterity. Typical Working Conditions: Work is primarily performed in an office environment or on the nursing units. Contact with residents, families, and other professionals, as well as all McLean staff. EMPLOYMENT REQUIREMENTS: Knowledge, Skills, Abilities: Knowledgeable and competent in the social work process within skilled nursing and assisted living environments. Knowledge of a wide range of disabilities and QI. Strong organizational and communication skills. Skill in establishing and maintaining positive working relationships with clients, families, and staff. Ability to maintain confidentiality of sensitive information. Ability to adhere to regulatory guidelines Education: Bachelor’s or Master’s degree in Social Work or Bachelor’s degree in related field.
Minimum of one year social work experience in SNF setting including rehab and long term care, and/or AL experience.
